§1
General provisions

  1. The security and confidentiality of your data is a priority for the Institute. Please be assured that we have always made, and will continue to make, every effort to protect your data by applying appropriate technical and organisational safeguards, including using appropriate protection when designing new services and solutions.
  2. These Regulations serve to inform customers and interested parties of the purpose, scope and categories of processing of their personal data, the duration of data processing and their rights, in accordance with the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (referred to as RODO/GDPR).
  3. In order to comply with the information obligation, information clauses adapted to the scope of personal data collected by the Institute are introduced.

§2
Information clause on personal data processing for students of postgraduate and doctoral studies


Referring to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2, hereinafter „Regulation”), the Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

  1. The Administrator of your personal data is the Institute of Soil Science and Plant Cultivation – State Research Institute (address: ul. Czartoryskich 8, 24-100 Puławy, tel. 81 4786 700 or 800, e-mail: iung@iung.pulawy.pl).
  2. The personal data administrator has appointed a data protection officer, with whom you may contact in matters concerning data processing and the exercise of rights related to data processing, through the e-mail address: iod@iung.pulawy.pl, telephone number – 81 4786 738, or in writing to the correspondence address of the personal data administrator indicated in point 1.
  3. The purposes of data processing and the legal basis for the processing:
  1. Period of personal data processing personal data will be processed for the period necessary for the realization of the above mentioned purposes taking into account the storage periods specified in separate regulations, including the period necessary for archiving. In the case of persons who have been admitted to studies, personal data will be processed for the purpose of concluding a contract with the Administrator and its performance or taking action at the request of the person who is the subject of the contract for the duration of the contract, and after its termination for a period of 50 years for archival purposes resulting from generally applicable provisions of the Law on Higher Education and Science and executive regulations.
  2. In the situation of the processing of your personal data on the basis of your consent, your personal data will be processed until you revoke your consent to its processing.
  3. Recipients of the data – the collected personal data may be made available to public authorities or entities entitled to obtain the data on the basis of universally applicable legislation, as well as to persons or entities that will participate in the implementation of the course of study; authorized employees of the Administrator may have access to your data.
  4. Rights related to data processing – you have the right to access your data, to demand their rectification, erasure or restriction of processing, to object to processing, to transfer your data in cases specified in the Regulation.
  5. Consent for the processing of personal data can be withdrawn at any time without affecting the legality of the processing performed before its withdrawal.
  6. In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.
  7. Information on automated decision-making and profiling – the Institute of Soil Science and Plant Cultivation – SRI in Puławy will not use your data for automated decision-making and profiling.
  8. Providing personal data in connection with the recruitment and participation in the course of studies is not mandatory, but the consequence of failing to provide personal data will be inability to participate in the studies.

§3
Information clause on processing of contractors’ personal data

Pursuant to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2, hereinafter „Regulation”), the Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

  1. The Administrator of your personal data is the Institute of Soil Science and Plant Cultivation – State Research Institute (address: ul. Czartoryskich 8, 24-100 Puławy, tel. 81 4786 700 or 800, e-mail: iung@iung.pulawy.pl).
  2. The Administrator of the personal data has appointed a Data Protection Officer, with whom you can contact in matters concerning the processing of data and exercise of rights related to the processing of data, through the e-mail address: iod@iung.pulawy.pl, telephone number – 81 4786 738, or in writing to the correspondence address of the Personal Data Administrator indicated in point 1.
  3. The purposes of data processing and the legal basis for the processing
    a) The personal data collected from you will be processed by the Administrator on the basis of Article 6(1)(c) of the Regulation (i.e. the need to comply with a legal obligation to which the Administrator is subject), in connection with the Accountancy Act, the Value Added Tax, and also on the basis of Article 6(1)(f) of the Regulation due to the Administrator’s legitimate interest (in particular to safeguard data for the purposes of pursuing potential claims).
    b) After concluding a contract, the legal basis for the processing of your data will be the performance of the contract (Article 6(1)(b) of the Regulation) to the extent specified in the contract. Personal data of contractors will be processed in order to provide services and perform the concluded agreement.
    c) With regard to data not required by law – the legal basis for processing shall be your consent (Article 6(1)(a) of the Regulation). Consent can be withdrawn at any time, without affecting the legality of processing performed before its withdrawal.
  4. Period of personal data processing – personal data will be processed for the time necessary to achieve the above mentioned objectives, taking into account the storage periods specified in separate regulations, including the time necessary for archiving, securing or pursuing potential claims to which the Administrator is entitled, for a minimum period of 5 years. In the case of processing your personal data on the basis of your consent, your personal data will be processed until you revoke your consent to its processing.
  5. Recipients of the data – the collected personal data may be disclosed to public authorities or entities entitled to obtain the data on the basis of generally applicable legislation, as well as to persons or entities that will participate in the performance of the contract; your data may be accessed by authorized employees of the Administrator, entities providing and maintaining the IT infrastructure of the Administrator, entities and persons providing legal services.
  6. Rights related to data processing – you have the right to access your data, to demand their rectification, erasure or restriction of processing, to object to processing, to transfer your data in cases specified in the Regulation.
  7. Consent for the processing of personal data can be withdrawn at any time, without affecting the legality of the processing performed before its withdrawal.
  8. In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.
  9. Information on automated decision-making and profiling – the Institute of Soil Science and Plant Cultivation – SRI in Puławy, will not use your data for automated decision-making and profiling.
  10. Providing personal data is necessary to establish cooperation with the Institute.

§4
Information clause on processing of personal data of participants of conferences, workshops, trainings

Pursuant to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2 hereinafter „Regulation” ) Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

  1. The Administrator of your personal data is the Institute of Soil Science and Plant Cultivation – State Research Institute (address: ul. Czartoryskich 8, 24-100 Puławy, tel. 81 4786 700 or 800, e-mail: iung@iung.pulawy.pl).
  2. The Administrator of the personal data has appointed a Data Protection Officer, with whom you can contact in matters concerning the processing of data and exercise of rights related to the processing of data, through the e-mail address: iod@iung.pulawy.pl, telephone number – 81 4786 738, or in writing to the correspondence address of the Personal Data Administrator indicated in point 1.
  3. The purposes of data processing and the legal basis for the processing
    a) Personal data collected from you will be processed by the Administrator on the basis of Article 6(1)(c) of the Regulation (i.e. the need to comply with a legal obligation to which the Administrator is subject), including the Act on accounting, tax on goods and services – in order to issue a VAT invoice.
    b) Personal data collected for the purpose of conducting training, your participation in workshops, conferences will be processed on the basis of your written consent (Article 6(1)(a) of the Regulation). Consent for the processing of personal data can be withdrawn at any time, without affecting the legality of processing performed before its withdrawal.
  4. Period of personal data processing – personal data will be processed for the time necessary to achieve the above mentioned objectives, taking into account the storage periods specified in separate regulations, including the time necessary for archiving, securing or pursuing potential claims to which the Administrator is entitled, for a minimum period of 5 years. In the case of processing your personal data on the basis of your consent, your personal data will be processed until you revoke your consent to its processing.
  5. Data recipients – collected personal data may be made available to public authorities or entities entitled to receive the data on the basis of generally applicable legislation, as well as to the supervisory authority – the Minister of Agriculture and Rural Development, persons or entities that will participate in the implementation of the conference, workshop, training; your data may be accessed by authorized employees of the Administrator.
  6. Rights related to data processing – you have the right to access your data, to demand their rectification, erasure or restriction of processing, to object to processing, to transfer your data in cases specified in the Regulation.
  7. Consent for the processing of personal data can be withdrawn at any time, without affecting the legality of the processing performed before its withdrawal.
  8. In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.
  9. Information on automated decision-making and profiling – the Institute of Soil Science and Plant Cultivation – SRI in Puławy, will not use your data for automated decision-making and profiling.
  10. Providing personal data in relation to your participation in conferences, workshops, trainings organized by the Administrator is not mandatory, but the consequence of failing to provide personal data will be the inability to participate in the above mentioned events.

§5
Information clause on processing of personal data for job applicants

Pursuant to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2 hereinafter „Regulation” ) Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

  1. The Administrator of your personal data is the Institute of Soil Science and Plant Cultivation – State Research Institute (address: ul. Czartoryskich 8, 24-100 Puławy, tel. 81 4786 700 or 800, e-mail: iung@iung.pulawy.pl).
  2. The Administrator of the personal data has appointed a Data Protection Officer, with whom you can contact in matters concerning the processing of data and exercise of rights related to the processing of data, through the e-mail address: iod@iung.pulawy.pl, telephone number – 81 4786 738, or in writing to the correspondence address of the Personal Data Administrator indicated in point 1.
  3. The purposes of data processing and the legal basis for the processing
    a) Your personal data, to the extent indicated in the provisions of labour law (Article 221 of the Labour Code and § 1 of the Regulation of the Minister of Labour and Social Policy of 28 May 1996 on the scope of keeping records by employers in matters relating to the employment relationship and the manner of keeping the employee’s personal file) shall be processed by the Administrator of personal data on the basis of Article 6(1)(b) of the Regulation in order to carry out the recruitment procedure.
    b) With regard to data not required by law – the legal basis for processing will be your consent (Article 6(1)(a) of the Regulation). Consent can be withdrawn at any time, without affecting the legality of processing performed before its withdrawal.
  4. Period of personal data processing – personal data shall be processed until the end of the recruitment process. In the case of your consent to the use of your personal data for the purposes of future recruitment, your personal data will be used for the following 6 months.
  5. Data recipients – Your data may be accessed by authorized employees of the Administrator.
  6. Rights in relation to data processing – you have the right to access the data, to demand their rectification, erasure or restriction of processing, to object to processing, to transfer the data in cases specified in the Regulation.
  7. Consent to the processing of personal data can be withdrawn at any time, without affecting the legality of processing carried out before its withdrawal.
  8. In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.
  9. Information on automated decision-making and profiling – the Institute of Soil Science and Plant Cultivation – SRI in Puławy will not use your data for automated decision-making and profiling.
  10. Providing personal data in the scope resulting from Article 221 of the Labour Code is necessary in order to participate in the recruitment procedure. Providing other data is voluntary.

§6
Information clause on processing of employees’ personal data

Pursuant to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2 hereinafter „Regulation” ) Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

  1. The Administrator of your personal data is the Institute of Soil Science and Plant Cultivation – State Research Institute (address: ul. Czartoryskich 8, 24-100 Puławy, tel. 81 4786 700 or 800, e-mail: iung@iung.pulawy.pl).
  2. The Administrator of the personal data has appointed a Data Protection Officer, with whom you can contact in matters concerning the processing of data and exercise of rights related to the processing of data, through the e-mail address: iod@iung.pulawy.pl, telephone number – 81 4786 738, or in writing to the correspondence address of the Personal Data Administrator indicated in point 1.
  3. The purposes of data processing and the legal basis for the processing
    a) Your personal data will be processed for purposes related to employment, including in particular for the purpose of fulfilling job duties, as well as for the purpose of performing the employer’s duties as an employer, payer of contributions and advance payments for income tax.
    b) The processing of personal data in connection with employment takes place on the basis of legal provisions, in particular tax law, labour law and social insurance law, including for the purpose of determining the employee’s capacity to work (Article 6(1)(c) of the Regulation, Article 9(2)(b) of the Regulation and Article 9(2)(h) of the Regulation).
    c) Personal data may be processed in connection with the assertion/defence of claims (Article 6(1)(f) of the Regulation, Article 9(2)(f) of the Regulation), as legitimate interest of the Data Administrator.
    d) With regard to data not required by law – the legal basis for processing shall be your consent (Article 6(1)(a) of the Regulation). Consent may be withdrawn at any time, without affecting the legality of the processing performed before its withdrawal.
  4. Period of personal data processing − personal data will be processed for the period required by law, including the period necessary for archiving and the period of limitation of claims related to employment (50 years) In the case of processing your personal data on the basis of your consent, your personal data will be processed until you revoke your consent to its processing.
  5. Recipients of the data – the collected personal data may be disclosed to public authorities or entities authorized to receive the data on the basis of generally applicable legislation, as well as to contractors on the basis of cooperation agreements concluded, within a consortium agreement, projects, in connection with public procurement law; authorized employees of the Administrator, entities providing and maintaining the Administrator’s IT infrastructure, entities and persons providing legal services will have access to your data.
  6. Rights related to data processing – you have the right to access your data, to demand their rectification, erasure or restriction of processing, to object to processing, to transfer your data in cases specified in the Regulation.
  7. Consent for the processing of personal data can be withdrawn at any time, without affecting the legality of the processing performed before its withdrawal. 8.
  8. In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.
  9. Information on automated decision-making and profiling – the Institute of Soil Science and Plant Cultivation – SRI in Puławy will not use your data for automated decision-making and profiling.
  10. Providing personal data resulting from Article 221 of the Labour Code is mandatory. Providing other data is voluntary, and not providing it may result in a lack of the possibility to obtain additional benefits, among others resulting from internal legal acts binding at the Administrator.

§7
Information clause on personal data processing – civil law contracts

Pursuant to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2 hereinafter „Regulation” ) Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

  1. The Administrator of your personal data is the Institute of Soil Science and Plant Cultivation – State Research Institute (address: ul. Czartoryskich 8, 24-100 Puławy, tel. 81 4786 700 or 800, e-mail: iung@iung.pulawy.pl).
  2.   3. The purposes of data processing and the legal basis for the processing
    a) Article 6(1)(b) of the Regulation – processing is necessary for the performance of a contract to which you are party – in order to perform the contract, or processing is necessary to take steps at the request of the data subject prior to entering into a contract – in order to conclude a contract,
    b) Article 6(1)(c) of the Regulation – processing is necessary for compliance with a legal obligation to which the administrator is subject – in order to fulfil legal obligations, e.g. as regards the storage of documentation relating to the conclusion of a contract; for accounting purposes,
    c) Article 6(1)(f) of the Regulation – legitimate interest of the Administrator, e.g. in order to secure data for future claims.
  4. Period of personal data processing – the period of processing of your personal data is the duration of the contract, and after its expiration for the period necessary to enable the service of the contract, securing or asserting possible claims and fulfilling the legal obligation of the administrator (e.g. resulting from tax or accounting regulations, archiving period).
  5. Data recipients – the collected personal data may be disclosed to public authorities or entities entitled to obtain the data on the basis of generally applicable legislation, as well as to persons or entities that will participate in the performance of the contract; your data may be accessed by authorized employees of the Administrator, entities providing and maintaining the IT infrastructure of the Administrator, entities and persons providing legal services.
  6. Rights in relation to data processing – you have the right to access the data, to demand their rectification, erasure or restriction of processing, to object to the processing, to transfer the data in cases specified in the Regulation.
  7. In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.
  8. Information on automated decision-making and profiling – the Institute of Soil Science and Plant Cultivation – PIB in Puławy will not use your data for automated decision-making and profiling.
  9. Providing your data is necessary to conclude and perform the contract, failing to provide them will prevent the conclusion of the contract and its settlement.

§8
Information clause on processing of personal data – video surveillance

Pursuant to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2 hereinafter „Regulation” ) Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

  1. The Administrator of your personal data is the Institute of Soil Science and Plant Cultivation – State Research Institute (address: ul. Czartoryskich 8, 24-100 Puławy, tel. 81 4786 700 or 800, e-mail: iung@iung.pulawy.pl).
  2. The Administrator of the personal data has appointed a Data Protection Officer, with whom you can contact in matters concerning the processing of data and exercise of rights related to the processing of data, through the e-mail address: iod@iung.pulawy.pl, telephone number – 81 4786 738, or in writing to the correspondence address of the Personal Data Administrator indicated in point 1.
  3. The purposes of data processing and the legal basis for the processing
    Your personal data in the form of image recorded by the monitoring system (monitoring involves real-time image analysis and recording) will be processed in order to protect property, ensure the safety of employees and other persons present on the Administrator’s premises, protect information the disclosure of which could expose the Administrator to damage, as the Administrator’s legitimate interest under Article 6(1)(f) of the Regulation.
  4. Period of personal data processing – materials obtained from the monitoring are stored for a period not exceeding 3 months from the date of recording. The storage time depends on the number of events and the capacity of the recorder. In the case where the recordings constitute evidence in proceedings conducted
    In the case where the recordings will constitute evidence in a proceeding conducted on the basis of legal regulations, the storage time shall be extended until the conclusion of the proceeding.
  5. Data recipients – the materials obtained from the monitoring will be available to authorized employees of the Administrator; the real-time image may be subject to analysis by security company employees. The collected personal data may be disclosed to public authorities or entities authorized to receive the data on the basis of generally applicable laws – public administration bodies, uniformed services.
  6. Rights in relation to data processing – you have the right to access the data, to demand their rectification, erasure or restriction of processing, to object to the processing, to transfer the data in cases specified in the Regulation.
  7. In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.
  8. Information on automated decision-making and profiling – the Institute of Soil Science and Plant Cultivation – SRI in Puławy will not use your data for automated decision-making and profiling.
  9. Your monitoring data is collected automatically – the monitoring system registers images continuously, therefore, being in the monitored area means acceptance of the necessity to provide personal data within the scope and for the purposes specified in point 3.

Area under monitoring
Administrator’s premises: at 8 Czartoryskich Street in Puławy (including the palace settlement); at 17 Aleja Królewska in Puławy; at 8 Krańcowa Street in Puławy; at 61 Orzechowa Street in Wrocław. Monitoring also includes the OSK Pulman building, the area of the Agricultural Experimental Station in Werbkowice (entrance to the palace at ul. Parkowa 6, workshops, livestock building, Kotorów 65 A farm), the area of the Agricultural Experimental Station Błonie-Topola (Błonie 138, 99-100 Łęczyca), the area of the Agricultural Experimental Station Kępa (ul. Kazimierska 4B, 24-100 Puławy), including a store building – monitoring inside and outside, Agricultural Experimental Station in Osiny (area around the pond, farm in Sadłowice), area of Experimental Station Wierzbno (66-340 Przytoczna), Wierzbno 11 (monitoring inside and outside the Hatchery), area of Agricultural Experimental Station in Wielichów (ul. Ziemińska 1, 64-050 Wielichowo).

§9
Information clause on personal data processing – GPS monitoring

Pursuant to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2 hereinafter „Regulation” ) Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

  1. The Administrator of your personal data is the Institute of Soil Science and Plant Cultivation – State Research Institute (address: ul. Czartoryskich 8, 24-100 Puławy, tel. 81 4786 700 or 800, e-mail: iung@iung.pulawy.pl).
  2. The Administrator of the personal data has appointed a Data Protection Officer, with whom you can contact in matters concerning the processing of data and exercise of rights related to the processing of data, through the e-mail address: iod@iung.pulawy.pl, telephone number – 81 4786 738, or in writing to the correspondence address of the Personal Data Administrator indicated in point 1.
  3. Purposes of data processing and legal basis of the processing – your data will be processed in order to control the proper use of the car, i.e. to ensure the organization of work allowing for the full use of working time pursuant to Article 6 paragraph 1 letter f of the Regulation.
    GPS monitoring will include: route and time of company cars, speed, location, vehicle odometer, number of driven kilometers, speeding.
  4. Period of personal data processing – materials obtained from GPS monitoring are stored for a period of 5 years. In the event that the recordings constitute evidence in proceedings conducted under the law, the storage period shall be extended until the end of the proceedings.
  5. Data recipients – access to the materials obtained from the monitoring is granted to authorized employees of the Administrator. The collected personal data may be made available to public authorities or entities entitled to receive the data on the basis of generally applicable laws. Data may also be processed by an entity with whom the Administrator has entered into an agreement for the operation of the GPS monitoring system in company cars.
  6. Rights in relation to data processing – you have the right to access the data, to demand their rectification, erasure or restriction of processing, to object to the processing, to transfer the data in cases specified in the Regulation.
  7. 7th In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.
  8. Information on automated decision-making and profiling – the Institute of Soil Science and Plant Cultivation – PIB in Puławy will not use your data for automated decision-making and profiling.
  9. Your data derived from the GPS monitoring is provided to the Administrator on a voluntary basis, but the lack of their processing will prevent you from driving vehicles forming part of the Administrator’s fleet for official purposes.

§10
Information clause on processing of personal data of retired employees and pensioners

Pursuant to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2 hereinafter „Regulation” ) Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

  1. The Administrator of your personal data is the Institute of Soil Science and Plant Cultivation – State Research Institute (address: ul. Czartoryskich 8, 24-100 Puławy, tel. 81 4786 700 or 800, e-mail: iung@iung.pulawy.pl).
  2. The Administrator of the personal data has appointed a Data Protection Officer, with whom you can contact in matters concerning the processing of data and exercise of rights related to the processing of data, through the e-mail address: iod@iung.pulawy.pl, telephone number – 81 4786 738, or in writing to the correspondence address of the Personal Data Administrator indicated in point 1.
  3. Purposes of data processing and legal basis of the processing
    a) Your personal data will be processed for the purposes related to your application for social benefits from the Company Social Benefits Fund, as well as to determine their amount (Article 6(1)(c) of the Regulation, Article 9(2)(b) of the Regulation).
    b) With regard to data not required by law – the legal basis for processing shall be your consent (Article 6(1)(a) of the Regulation). Consent can be withdrawn at any time, without affecting the legality of the processing performed before its withdrawal.
  4. Period of personal data processing − personal data will be processed for the period required by law, including the period necessary for archiving. In the situation of the processing of your personal data on the basis of your consent, your personal data will be processed until you revoke your consent to its processing.
  5. Data recipients – the collected personal data may be made available to public authorities or entities authorized to receive the data on the basis of universally applicable laws; authorized employees of the Administrator, entities providing and maintaining the IT infrastructure of the Administrator, entities and persons providing legal services will have access to your data.
  6. Rights in relation to data processing − you have the right to access your data, to demand their rectification, erasure or restriction of processing, to object to processing, to transfer your data in cases specified in the Regulation.
  7. Consent for the processing of personal data can be withdrawn at any time, without affecting the legality of the processing performed before its withdrawal. 8.
  8. In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.
  9. Information on automated decision-making and profiling – Institute of Soil Science and Plant Cultivation – SRI in Puławy will not use your data for automated decision-making and profiling.
  10. Providing your personal data for the purposes related to your application for social benefits from the Company Social Benefits Fund, as well as to determine their amount is a statutory requirement, and the consequence of failing to provide personal data is the inability to grant benefits from the Company Social Benefits Fund.
    In the scope of personal data that would be processed on the basis of your consent, its submission is voluntary.

Information clause on processing of data of family members of employees

Pursuant to the content of Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) – (OJ EU.L 119 of 04.05.2016, p. 1 and OJ EU L 127 of 23.05.2018 p. 2 hereinafter „Regulation” ) Institute of Soil Science and Plant Cultivation – SRI in Puławy informs that:

The legal basis for the processing will be your consent (Article 6(1)(a) of the Regulation). Consent for the processing of personal data can be withdrawn at any time, without affecting the legality of the processing performed before its withdrawal.
  4. Categories of personal data
    The Administrator will process your personal data to the extent indicated in the employee’s declaration of coverage under medical packages: name and surname, pesel, date of birth, residential address, degree of relationship with the Administrator’s employee.
  5. Source of personal data
    Your personal data will be transferred to the Administrator by an employee of the Institute of Soil Science and Plant Cultivation – SRI in Puławy, who is your family member, by submitting a declaration of medical care within medical packages.
  6. Period of personal data processing − personal data will be processed for the period of performance of the contract concluded between the Administrator and the company providing medical services or until you revoke your consent to its processing.
  7. Data recipients – the collected personal data will be made available to the company with which the Administrator has concluded an agreement on medical care within the framework of medical packages, to public authorities or entities entitled to obtain the data on the basis of universally binding provisions of law; authorized employees of the Administrator, entities providing and maintaining the Administrator’s IT infrastructure will have access to your data.
  8. Rights related to data processing – you have the right to access your data, to demand their rectification, erasure or restriction of processing, to object to processing, to transfer your data, in cases specified in the Regulation.
  9. Consent for the processing of personal data can be withdrawn at any time, without affecting the legality of the processing performed before its withdrawal.
  10. In case of recognition that the processing of personal data violates the provisions of the Regulation, you have the right to lodge a complaint to the President of the Office for Personal Data Protection.

A form has been made available on the Institute’s website, with which you can apply for the exercise of your rights.
Completed applications may be submitted in person or by mail to the address of the Institute’s headquarters or by e-mail to: iod@iung.pulawy.pl by sending an electronically signed application or a scan of a manually signed application.
Each application submitted by you is considered individually and in accordance with the applicable law. We would like to remind you that the possibility of exercising a particular right may depend on the legal basis used for the specific purpose of processing your personal data and may depend, for example, on the fact that the processing of data is not dependent on the performance of a contract or a service.
Your requests will be processed without undue delay and within a maximum of one month of receipt. However, it is possible that due to the nature of the request we will not be able to meet this deadline, in which case you will be informed of the delay and the reasons for it. Similarly, should such circumstances arise, we will inform you of the reasons why we refuse to accept and process your request.
You will be informed promptly of the reasons why we are unable to process your request.
In addition, in order to guarantee the security of the information transmitted, in the event that we are unable to identify you correctly as the person authorised to receive the data, we reserve the right to change the manner in which the information is provided, of which you will be informed.
If the right to data portability is exercised, the Data Administrator will directly transfer the data to another data administrator, if technically possible. The person requesting the exercise of this right will be informed of the decision regarding the possibility of data portability.

§ 13
Final provisions

These Terms and Conditions are valid from 08.03.2021 until further notice and implement the legal obligation arising from Articles 13-14 of the Regulation . We reserve the right to amend these Regulations, always with the aim of improving the quality of our services and respecting your rights and privacy.
